What to Expect from a PSMA PET Scan: A Patient’s Guide

What Is a PSMA PET Scan?

PSMA stands for Prostate-Specific Membrane Antigen—a protein that is often found in high amounts on prostate cancer cells. A PSMA PET scan is a highly advanced imaging test that uses a small amount of radioactive tracer to detect prostate cancer, especially in cases where the cancer may have returned or spread to other parts of the body. This scan provides greater accuracy than traditional imaging and can help your medical team make informed decisions about your treatment plan.

Why You Might Need One

Your doctor may refer you for a PSMA PET scan if:

  • Your PSA levels are rising after prostate cancer treatment
  • You’ve recently been diagnosed with prostate cancer and your provider wants to see if it has spread
  • You’re being evaluated for advanced or metastatic disease
  • You are being considered for targeted therapy or surgery

How to Prepare

The good news is that preparing for a PSMA PET scan is relatively simple. Here’s what you need to know:

  • Fasting: You may be asked not to eat or drink anything except water for a few hours before your scan. Our team will give you specific instructions.
  • Medications: Most medications can be continued as usual, but always check with your doctor or the scheduling team ahead of time.
  • Hydration: Staying well hydrated can help your body eliminate the tracer after the scan. Drink plenty of water before and after the exam.
  • What to Wear: Wear comfortable clothes and leave jewelry at home. You may be asked to change into a gown.
  • Bring Support: You’re welcome to bring a support person, but they may be asked to wait in the lobby during the scan.

What Happens During the Scan

The process typically takes about 1.5 to 2 hours total:

  1. Injection of the Tracer: First, you’ll receive an injection of the PSMA-targeting tracer. This tracer needs time to circulate through your body—usually around 60 minutes.
  2. Waiting Period: After the injection, you’ll wait in a comfortable space until it’s time for the scan.
  3. The Scan: You’ll lie on a flat table that slowly moves through the PET/CT scanner. The scan itself takes about 20–40 minutes. It’s painless and quiet—just be sure to lie still.
  4. Afterward: You can go about your day as normal. We recommend drinking water to help flush the tracer from your system.

When Will I Get Results?

Your scan images will be reviewed by a subspecialized radiologist experienced in PSMA PET imaging. A detailed report will be sent to your referring provider—usually within 48 hours. Your provider will then review the results with you and talk through the next steps.
